This happens e.g. if a field is named index or primary. The same applies for the database name and table names.

[--]      +-- Column mytable.primary:
[!!] failed to execute: SELECT primary FROM mydb.mytable PROCEDURE ANALYSE(100000)
[!!] FAIL Execute SQL / return code: 256
