logout tab #158

on selection of the logout the following error is given:

Not Found

The requested URL /user/logout was not found on this server.
Apache/2.4.7 (Ubuntu) Server at midas Port 80

presumably trying to resolve index.php/user/logout

I cannot reproduce this locally and it is not evident on any production instances that I manage.
