Provide more info modal repeats screen reader description

Steps to Reproduce

Using a screen reader, attempt to enroll in a course with a user who has Addl field flag equal to False. The "Provide more info" modal should display when you attempt to visit the course.

Expected Behavior

The screen reader should read each part of the form as the user navigates between elements. The description of the modal should not be repeated as the user navigates.

Actual Behavior

When navigating between elements in the modal, the screen reader will repeat the description of the entire modal.

@pdpinch I took it to have something to do in between stuff - haven't started on it yet and the course/program about pages are priority ahead of this.
