Medical Conditions Module changes

"..changes to the illnesses and medical conditions module - I realised that for some sections, there is more than one medical condition listed (e.g. pneumonia and other respiratory problems). I listed extra status questions (highlighted in the attachment), which would need to have corresponding status questions listed in the longitudinal follow up module (which I can add once Meagan ok's)."

This is probably definition change.
