Users with elevated roles events are not triggering except if a cache clear happened recently

This might be due us using Rules, which adds a whole bunch of complexity. We might be able to fix it by refactoring so we no longer use Rules and act on Drupal core hooks alone.

An alternative hypothesis is that this is caused by relying on sendmail which is sometimes unreliable on some hosts which has little to do with this module.
