• Root cause: I added the playwright MCP server using the GitHub URL https://github.com/microsoft/playwright-mcp, so Codex tried to speak over HTTP and hit a 422
  Unprocessable Entity during handshake.
• Fix: Removed that entry and re-added the server with codex mcp add playwright -- npx @playwright/mcp@latest, which launches it via npx over STDIO.
• Versions: Codex CLI codex-cli 0.46.0, MCP server @playwright/mcp version 0.0.42.
