Syntax Error when adding order history

When attempting to add history to an order, an error pops up:

Syntax Error: Unexpected end of input

a few things to try.

make sure maintenance mode is off.

make sure the web site is not password protected to access.

if you have upgrade make sure the config.php has the correct path to the front of your site.

this includes under stores.
