Add Indexer Logs, add manual index requests

For this ticket, we'll make a few updates to improve our indexing:

1. More detailed logs
2. Allow manual admin requests to start reindexing a particular chain from a particular block
3. Make StakeEvent unique on tx hash
4. Do updates in a db transaction, only updating the summary if the event is successfully logged
