Has anyone ran into a situation where versions are out of order?

I'm not sure how this happened, but I ran into a situation where I was saving a document, and those changes weren't appearing on the list view or when trying to filter. Upon further investigation, I found our versions were a bit out of order, as seen in the screenshot.

(Ignore the top version for a moment)

My changes were being saved to the 819 / 4:23pm version
But the 820 / 1:05pm was the newest version

After noticing this, I was able to revert to the 819 version, and that's what created the 825 version.

I'm using autosave / versions / drafts on postgres

Any ideas on what might have happened?
