Create usage instructions and rearrange docs

The documentation section on Extending and reusing setuptools (http://pythonhosted.org//setuptools/setuptools.html#extending-and-reusing-setuptools) contains a lot of usage instructions which should be moved to the usage instructions section which is currently not worth mentioning (http://pythonhosted.org//setuptools/using.html). I didn't yet manage to get a project running (because of the annoying search of documentation here and there), but I'd guess that from the subsection http://pythonhosted.org//setuptools/setuptools.html#extensible-applications-and-frameworks on the Extending and resuing section actually contains instructions for developping, i.e. extending and reusing.
