Add Polaris HEVC encode

• Update SDK to AMD Media Framework 1.4.
  • AMD Radeon Softwawre Crimson 17.1.1 or later is required.
• Add support for Polaris HEVC encoding. (-c hevc)
• Add SAR options. (--sar <int>:<int>, --dar <int>:<int>)
• Add pre analysis option. (--pre-analysis <string>)
• Add VBAQ option. (--vbaq, H.264 only)
• Fix some problems from previous version.
