UI looks broken for Nginx Map Section #160

In the Admin Panel when Nginx Map is enabled, the Nginx Map section looks broken

You can see that in the screenshot above.

For ref: I have added the current Nginx Map Section as well below.

I'm seeing the same thing. It doesn't create the map.conf file either.

I'm on a WP Multisite btw.
