Use templates for Sardana issues #31

We should start using issue templates (check how spyder project does it)
Templates could contain space for: sardana version, dependencies versions (ZMQ, Tango, Taurus), OS, expected behavior, observed behavior
We should then provide script for checking dependencies versions (Taurus has something similar)
