I am facing this error in the collect app.

when i try to get black from in the collect app, i am getting this error. can any one guide me with this error.

@cooperka here are server logs. i am unable to download form which contains nested options set.

@cooperka Form Appears in the Get Blank Form list but fails to download.

hmm, the log file screenshot seems to show only successful requests -- i don't see any error message so i can't help much, and unfortunately the NEMO developer team only has limited time and is not paid for software support outside of The Carter Center's operations.

please also be aware that any info you share here is publicly available to anyone, so you may want to diagnose this with a trusted software developer if there's any sensitive information.

@cooperka when i try to download the from in collect app it's throwing unauthorized error in the collect app and also here on the server, i am using valid credentials in the collect app but still it throws this error
