[DOC]: Add issue template to repository

I'm writing this issue to request that an issue template be added to our repository. Having a template for creating new issues can help streamline the process and ensure that important information is included in each issue.

The template should include sections for the following information:

• A brief summary of the issue
• Steps to reproduce the issue (if applicable)
• Expected behavior
• Actual behavior
• Environment information (e.g. operating system, browser, device)
• Any relevant logs or error messages

Additionally, we can add some guidelines for issue reporting to the template, such as searching existing issues before creating a new one, providing screenshots or videos when applicable, and being descriptive and concise.
