Publish Form creates Table with multiple blank rows

Current behavior

When a user publishes a form the table created (sometimes) has multiple (random number of) blank rows.

Expected behavior

When a user publishes a form the table created shows "No data available in table"

Steps to reproduce the behavior

1. Publish a Form (activity/forms) in Activity
   

2. Load the Table in Track (/activity/forms/###/view)

3. view blank rows. If your table does not have blank rows, repeate step 1 and 2 until you see a Table with a random number of blank rows.
