This is deliverables D4.2 from the integration proposal. Backends to support include:

JSKOS API (aka Cocoda but Cocoda only makes use of this API)
EMBL-EBI Ontology Xref Service (OxO) (see source code)

There is also SSSOM api but not sure whether this is only a prototype not used in practice.

Requires:

Create JSKOS-API Backend #51 to build on
maybe Create mapping from/to SSSOM gbv/jskos-tools#44 to find a common data model of mappings (although JSKOS Concept Mappings model 0.6.0 should already be enough except for additional field justification to be introduced with Add mapping field: justification gbv/jskos#148 to align with SSSOM)

I would prefer to stay as close to JSKOS API as possible at least for mappings.
