Rare file attachment issue permission issue

If a file attachment is linked to two answers and one is inactive then students will not be able to see the file attachment of the active one.

This occurred in one class for one student INC2277805 INC2276833

currently not sure how an answer used the same file attachment twice but setting the inactive answer's file_id to null solved the issue for the students
