Add Difficulty Name of Map to Output Name

The difficulty name of a replay is in its file name, so it should be possible to take it and add it to the output name. This only really matters if you have multiple replays from the same mapset with the same acc that were set on the same day that you want to convert, granted you could edit the video names after they have been converted.
