Make address history button optional

We have logging enabled & when we enable this extension we get an address history button. This is kind of confusing because we have already an address history tab enabled via the extended reports extension which is configured to be more useful for us & having something that appears unrelated to the extension pop up as a result of installing it is a bit problematic

(The extended reports extension has a feature allowing reports to be enabled as tabs on the contact summary screen - this means we can choose the columns we want which are a bit different to yours. The address history report also retrieves information about the address history of contacts that have been merged into the current contact so it spans merges - although the way that is done is still a bit open to change)
