Today, we display basic vindex information on this vtctld web UI page. However, we have added significant additional vindex options since this page was last updated (e.g. for lookup vindexes); and this is not reflected on this page.

We should add that.

given that hash vindexes can be shared by multiple tables, the table column vindex associations need to be displayed somewhere.
currently, the table popup has Vindex and Sequence columns - but these are empty (even for indexed/sequenced table columns). this needs to be fixed.
