fix(docs): inter-linking was broken

- main navigation links were broken
- goin from index.html to any sub-directory worked fine, but going back
  though the navigation menu was broken
- from example from index.html to docker/docker.html was OK, but
  the link back changed to docker/index.html instead of ../index.html

- README was really out-of-date at this point, so here is an update
- fixed few things here and there in the getting started section
