Don't allow users to create blank Issues
* Add additional instructions and details on what users should enter.

Users are bypassing the Issue Template which asks them to use the Discussions link for questions, and are entering questions as Issues. This PR should force them to use the template, which was my intention all along.
This PR also adds details to the Issue Template so we (hopefully) get more information.
