New Code Tables for Identification Attributes

With the approval of #4829 a code table will be needed to record types of identification attributes.

Purpose	Table	Description
Identification: Identification Attribute Types	ctidentification_attribute_type	Controlled vocabulary for identification attribute types.

in addition, a table linking some values with controlled vocabularies will be needed.

Identification: Attribute Code Tables	ctidentification_attribute_code_tables	Each identification attribute that is controlled by a code table is listed here along with the code table that includes the controlled vocabulary.
