Adds a dedicated issue template for reporting problems on learn.microsoft.com-hosted API ref pages and READMEs.

This new feedback control will replace the existing one:
When clicking Open a documentation issue, the new issue template added in this PR will populate with the appropriate metadata. One benefit will be that such issues are created in this engineering repo, where the content lives.
