AMS not used by all cages, make additionalNameGiven always visible #39

Older cages may not have an AMS number.

Also AMS cage names are stricly a McGill cage identification system.

Solution

make a select field for cage identification (AMS/Other)
reveal mandatory id entry

Have a additionalNameGiven field that is optional and always visible, remove the cageNameGiven field as it is redundant.
