Fixes issues with the Audit Information form requiring things when the relevant checkboxes were not checked.

To check this, compare the behavior when filling out the form on dev versus on this branch.

This doesn't have tests; this is correcting an error that slipped through because of a prior lack of tests. Tests may be added in a future PR.

PR checklist: submitters

• If Git surgery is necessary at any point (such as during a merge party), repeat the testing after it’s finished.

The larger the PR, the stricter we should be about these points.
