Reproducible on kotlinx.serialization.

Consider the following TOC:

<!--- TOC -->
<!--- END -->

### Maven/JVM

Expected result:

<!--- TOC -->
* [Maven/JVM](#mavenjvm)
<!--- END -->

### Maven/JVM

Actual result:

<!--- TOC -->
* [Maven/JVM](#maven/jvm) // <- broken md link
<!--- END -->

### Maven/JVM
