Persisting cohort entry across multiple observation periods

Say a person has two observation periods and in the first they have a diagnosis of diabetes but no such record in the second. Is there a way to define a diabetes cohort so that they have one entry from the diagnosis date until their first observation end, and a second cohort entry from their second observation start to second observation end?
