Create post-committee form

This form should allow the volunteers to:

• mark the transactions that:
  • approved / אושר
  • approved + required revision / אושר + רוויזיה
  • discussed but not voted / נידון אך לא עלה להצבעה
  • not discussed and not voted / לא נידון ולא הוצבע
  • denied / לא אושר
• correct the date of the committee meeting for that specific transaction
