Add `output_transform` example for decollated data in handlers

Thanks for the feedback from internal team:
To get expected data from MONAI engine, we usually use `output_transform` of handlers. But as the data may be decollated `list of dicts`, need to add some example lambda function in the doc-string to enhance user experience.
