add from ext. display name to snippet tooltip title (#15)

- make snippet prefix and name bold
- add horizontal rule for the snippet tooltip title to better separate it from snippet body

title now has ext. name it came from and bold prefix and snippet name with a ruler to make snippet tooltip titlebar stand out more:
