[BUG] MCP tool action asks for approval even if set to auto-approve

When using an MCP tool action with auto-approve set, Roo still asks for approval. This seems to be a regression, and occurs in version 3.31.1

Reverting to version 3.31.0 behaves correctly with respect to MCP tool auto-approval.

Context (who is affected and when)

Whenever utilizing an MCP tool, auto-approve seems to be ignored.

Reproduction steps

1. VSCodium (latest), Roo version 3.31.1
2. Construct a prompt that triggers any MCP tool action that has auto-approve set to true

Expected result

The MCP tool task will execute without asking to approve the action

Actual result

Roo prompts to approve the MCP action, even though auto-approve is set to true

I'm exploring a potential fix for the MCP tool auto-approve regression and will open a PR to share progress.

Opened a PR with my attempt at addressing this issue: #9205

The fix restores the previous behavior where setting alwaysAllowMcp: true auto-approves all MCP tools by default, while still allowing individual tools to opt-out by explicitly setting alwaysAllow: false.
