Enhance case data form for IMI specific

Feature Description

After the Disease enum has been modified in (#13346) as part of epic (#13345), the case data form needs to be updated with IMI-specific details.

Acceptance Criteria

• Notification box should be visible, if its notified by MyGuichet.
• Info Button with the classification details as
  • Suspect Classification
    • Any person meating at least one of the following symptom
      • MENINGEAL_SIGNS
      • HEMORRHAGIC_RASH
      • SHOCK
      • ARTHRITIS
  • Probable Classification
    • Any person meeting the suspect criteria with epidemiological link to a confirmed case
  • Confirmed Classification
    • Any person meeting the laboratory criteria
      • MICROSCOPY
      • PCR_RT_PCR
      • ANTIGEN_DETECTION
  • Not a case Classification
• Pre-existing conditions hidden.
• Additional medical information section should be visible.
• General comment visible
• Paper form dates hidden.
