Bug Description

Steps to Reproduce

  1. Create a case on mobile app, the first form does not match the first form on the web

Expected Behavior

-Rename facility type group with facility category

  • When facility type == hospital, set the following default values, just as on the web:
    facility category = medical facility and facility type = hospital
