First multi-image ico

First attempt at creating a multi-image ico file, containing all the
separate icon sizes 16x16 - 256x256. Unclear if this works as expected,
as simply resizing the folder view in Windows doesn't seem to trigger
the different sizes (so either the ico file isn't correct, or Windows
makes it unnecessarily difficult here)
