at james' suggestion adding a feedback feature directly to the schedule app would make attendee access to said form easier

option 1: external link to google forms
option 2: create a menu page for feedback
- on submit push data to some structureless database (e.g. s3, pushing directly onto a google sheets submission form if able)
- validate all forms prior to submit
- save state of form if user enters and exits form
- optional: external validation with eventbrite to check that user is an actual registered user

issue is dependent on #60, #62, and probably #63
