Currently we are already handling the case when a liveagent transfers a chat to a second agent, using the 'transfer chat' button in the SF UI.

However, when the chat gets transferred to a new agent, because of the AgentDisconnected issue, there is a different mechanism going on, and our widget does not pick up the new agent name in the banner or in the avatar.

We need to fix.
