Fixed, but it appears that that the keyboard shortcuts displayed aren't correct.

I see "[CMD]=" and "[CMD]". Technically, "[CMD]=" is correct. Looks like "[CMD]-" has issues with the minus. However, apps like Chrome actually display "[CMD]+" as the shortcut label instead of using the equals sign. I'll fix "[CMD]-" as my first priority and see what would be involved to show the plus sign instead of the equals sign.
