Add Form Name and Form ID new columns in analysis conversion report #9

The analysis conversion report is currently missing the form name and the form id information in independent columns. The form name could be detected by parsing the item paths, but it could be confusing if the forms are nested in sub-folders. It is better to add two new columns to facilitate the use of the report.
