Remarks on the proposed questions:

What JavaScript stylistic choices do you prefer?

Provide visual examples, or remove this question. Also, the "other" option should be available.

I have my style, but I couldn't give any answer to such a question, without a visual example.

Static Analysis: Do you use any tools for verifying code quality?

What if someone use no tools? Such a question should be optional.

How do you write client-side scripts?

A "Plan old files" option should be also available.

Project Discovery: How do you find reusable code, libraries and tools?

Add echojs, or, better, "Other sources".

For each "other" option, a text field with participant's explanation should be available.

For the libraries/frameworks question suggested by others, I would opt for some distinction between use in private projects, and use at work. I cooperate with a company, which uses framework X (the actual name of the framework is not important) - yet I wouldn't ever used this framework in my private projects, and I don't.

Proposed questions:

Where do you use JavaScript? (multiple choice)

  • At work.
  • In my pet-projects.
  • Other.

How much of your development time do you use JavaScript?

  • 80% - 100% (occasionally using other languages)
  • 60% - 80% (primarily)
  • 40% - 60% (often)
  • 20% - 40% (sometimes)
  • 0% - 20% (occasionally)

How long have you been using JavaScript? (radio)

  • 1 year or less (a novice, a newcomer)
  • 1-2 years (joined the JavaScript hype of the recent years)
  • 3-5 years (predicted the hype ;-), started using node.js)
  • 5-10 years (an old fellow)
  • more than 10 years (a founding father ;-))

Time periods can be changed. In parenthesis, I gave some explanations (my own interpretations), which don't have to be included in the survey options.

Thanks, those are some good points. I think I had a gist for the code style examples, so I'll try to find it.
