dynamically registered MCP servers are not presented in TUI status

Description

i was working on #7275 and realized that while i had successfuly registered the vscode-mcp-server via the server http api it was still not presented in the tui mcp server count or status list.

Steps to reproduce

1. launch opencode
2. register an additional mcp server via the server http api
3. note that the new mcp server

• works
• does not increment the mcp count in the tui
• is not listed in the mcp status list in the tui

This issue is related to the work being done in #7275. The bug was discovered while implementing dynamic MCP server configuration. It would be helpful to resolve this issue as part of the broader feature work in #7275.
