Use :channels namespace in Appsignal.Phoenix.Channel

Closes #194.

Instead of using the :background_job namespace,
Appsignal.Phoenix.Channel will use :channels instead. This also allows
users to set their own custom namespaces in
Appsignal.Transaction.start/2.
