Enable the pagenums attribute by default so that users are aware of the running header / footer capability. They can be disabled either by disabilng this attribute or using blank content in the theme file.

I think we should consider activating the running header/footer by default (with or without the pagenums attribute) and allow it to be disabled using the existing noheader and nofooter attributes.
