Acceptance criteria

external api should have a field called "Middle Name"
"Middle Name" must be available in /api/subjects and /api/subject/{id} if it is configured
Documentation is updated to contain middle name
Create and update of subject (POST and PUT of /api/subject and /api/subject/{id}) must contain "Middle Name" parameter
POST/PUT will accept middle name only when middle names are enabled for the subject type
