Action icons overlap session title in chat history on hover

Describe the Bug

In the History section, past chat sessions display a title along with action icons (such as edit). When hovering over these sessions, the edit icon overlaps with the session title, especially if the title is long

To Reproduce

1. Go to the History section.
2. Hover or scroll over a past session/chat entry.
3. Observe that the edit icon overlaps with the session title.

Expected Behavior

The edit icon should be positioned properly and should not overlap with the session title, regardless of the title length.

I actually noticed it once.. thought it was a bug...but never happened again.. it really was a bug :)

It was a minor bug. I also didn’t notice it at first, but at different zoom levels, it starts to break
