rescanprogress/finished notifications must include block hash #151

The two progress notifications originating from the rescan RPC, rescanprogress and rescanfinished, currently include only the block height of the last processed block. These should be changed to also include the block hash, so different blocks on different forks with the same height can be distinguished.
