Creating as a separate issue to break-up issue #2

Finalizing the Severe Obesity Phenotype requires:

Questions:

  1. Case criteria requires presence of a height measurement greater than 5th percentile. How will we know this?
  2. Am I missing any steroids (common names not shown)?

Closing issue, for now, we will focus on phenotypes for both adult and pediatric populations. We may re-open this for testing though so I will keep the work done so far.
