Investigate moving the custom fields inside the main form

Even though the fields does its own persistence and will save independantly of the form it is inside of I thin this will be better ux because its all in one place. At the moment you make a new template, then turn on fields, then save the form, then edit the fields, then leave and go back to the template and rinse and repeat until you have all the fields you need. You be much nicer to just save and reload in place and keep going
