Use more unique page anchors

See discussion in #147. Currently page anchors on multi-declaration pages are simple indexes. Changing the oder of itens or adding a new item in-between will invalidate the anchors in permanent links. It would be nice to have a definition where at least in simple cases the anchors will stay stable.
